Understanding pregnancy as a set of stories

Pregnancy is rarely only a biological or medical event - it is a thick knot of stories about identity, responsibility, body image, family expectations and cultural scripts. You might find yourself repeating phrases you heard as a child, echoing medical language that feels unfamiliar, or carrying hopeful and fearful narratives at the same time. Narrative therapy, which emerged in Australia and New Zealand in the 1980s through the work of Michael White and David Epston, starts from the idea that people make sense of their lives through stories. That means the difficulty you bring to sessions is often best understood as a story - or several competing stories - rather than a single label.

When a therapist works with a narrative approach, they aim to make those stories visible so you can examine where they came from, which parts are helpful, and which parts narrow your sense of who you are. The focus is not on proving a diagnosis or teaching a fixed set of skills. Instead, it is on exploring how cultural expectations, family messages and personal history have shaped the story you are living. That opens space for different storylines to appear - moments and experiences that do not fit the dominant narrative - which can be gathered and amplified to create a preferred story you can live by.

How narrative therapy works with pregnancy

In practice, narrative therapy uses specific conversational tools to shift the relationship you have with the stories you carry. Externalising is central - naming the problem as something you experience rather than something that is you. For example, rather than saying "I am anxious," a narrative approach might invite you to explore the shape of the anxiety as an influence - when it arrives, how it behaves, and where it asks you to act in certain ways. This separation makes it possible to describe the problem in detail, examine its effects, and choose responses without equating it with your identity.

Another key practice is tracing the origins of a familiar story. You and your therapist might look together at when and how particular expectations about pregnancy were formed - shared sayings in your family, medical encounters, social media narratives, or workplace pressures. Thin description - the abbreviated, one-dimensional stories that flatten experience - is identified and contrasted with richer, fuller descriptions that include context, exceptions and values. Gathering unique outcomes means noticing moments during pregnancy when you acted differently from what the dominant story would predict - small events that point to a different possible future. Over time, these elements feed into re-authoring conversations and, where appropriate, re-membering work that brings forward relationships and values important to the story you prefer to live.

What to expect in narrative sessions for pregnancy

Sessions that use a narrative approach tend to be conversational and investigative. Early meetings often focus on naming the problem and externalising it - asking you to describe the issue as something with a life and influence. Your therapist might ask questions about when the story began, who or what contributed to its shape, and where it has most influence in your daily life. You can expect close attention to language - the words you use to describe yourself, your body and your pregnancy are treated as informative guides to how a story is assembled.

As the work progresses, conversations move toward gathering exceptions and developing richer descriptions. Your therapist may invite you to recall specific moments that contradict the dominant narrative - times you felt calm, competent, seen, or connected. Those moments are treated as evidence for alternative plots. Later sessions may include structured conversations aimed at re-authoring - rehearsing new ways of talking about your pregnancy that foreground your values and commitments. Some therapists also use re-membering conversations, where relationships that matter to you are invited into the work in carefully considered ways, and definitional ceremonies, where preferred meanings are acknowledged by others. It is important to know that many therapists who list narrative therapy among their approaches combine it with other methods. How much of a session is explicitly narrative versus integrative varies by clinician, so asking about their usual session mix is useful.

Is a narrative approach the right fit for pregnancy?

People are drawn to narrative work for different reasons. You might prefer it if you do not want to be reduced to a diagnostic label or if you find that a story handed down by family or culture no longer fits your experience. Narrative work is often appealing to those who want to explore values, reshape identity and make room for new possibilities rather than only learning techniques or seeking insight into the past. Compared with approaches that focus primarily on thought content, such as cognitive-behavioral methods, narrative therapy gives more weight to language, social influence and authorship of life stories. Compared with skills-oriented work, it places less emphasis on training specific behavioural tools and more on reshaping meaning. Compared with psychodynamic approaches that focus on unconscious motivations and early relationships, narrative practice concentrates on the stories you can describe and change in the present. None of these differences implies that one approach is superior; rather, they reflect different starting points and emphases.

There is no reliable way to tell in advance whether a narrative approach will suit you. Many people use an initial conversation to get a sense of a therapist's style and whether their way of working feels right. It is common for narrative approaches to be blended with other methods, especially when practical skills, symptom management or medical liaison are part of the support you need during pregnancy.

How to choose a therapist who works narratively with pregnancy

When you explore therapist profiles, focus on the areas that matter most to you: how much of the work is narrative, the therapist's experience with pregnancy or perinatal issues, languages offered, and relevant professional credentials or registration where you live. It is reasonable to ask a therapist directly how much of their practice uses narrative methods and what a typical session looks like. You can also ask about their experience with conversations like externalising, re-authoring or outsider witness practices. Remember that listing narrative therapy among approaches does not in itself confirm any specific training or credential, so questions about where they trained and how they describe their use of narrative are appropriate.

If you are considering online work, narrative conversations translate well to video because the work is largely talk-based and relational. You might ask how they structure online sessions and whether they use any written or creative tasks between meetings. In a first conversation look for whether the therapist invites your questions, listens to how you describe your story and offers examples of what narrative work looks like in practice. That exchange is often the best indicator of whether you will feel able to develop a preferred story with that person. Finally, make sure to verify licensure or registration rules that apply where you live, and feel empowered to follow up on anything you need to understand before starting work together.

Most people find that a single conversation can clarify whether the therapist's approach resonates. If you choose to proceed, narrative work usually unfolds over time - gradually enlarging the range of stories you can draw from and making space for the values and commitments that matter most to you during pregnancy and beyond.
