Understanding compassion fatigue through a narrative lens

Compassion fatigue often arrives as a story you tell about what caring costs you and what it means to be the kind of person who helps. That story can be shaped by workplace expectations, cultural ideas about caregiving, family messages, or the routines of your profession. Narrative therapy, developed in Australia and New Zealand in the 1980s by Michael White and David Epston, begins from the idea that people make sense of their lives through stories. From this perspective, the distress you experience is entangled with the narratives you have about responsibility, limits, identity and worth. The task is not to argue symptoms away but to explore the story and the roles it gives you, so that other, more livable stories can be noticed and strengthened.
When you look at compassion fatigue narratively, you move attention from a label toward the meanings and events that gave shape to that label. Instead of seeing an unchangeable trait, you can describe patterns - the expectations you absorbed, the moments when you said yes despite exhaustion, the times others relied on you without noticing your cost. Naming these patterns opens up space to examine them, trace their origins, and look for places where different choices or values have already appeared.
How narrative therapy works with compassion fatigue
Core practices applied to everyday caring
A narrative approach offers specific practices that shape how the issue is worked with. Externalising language is central - you and a therapist talk about compassion fatigue as something that affects your life, not as the entirety of who you are. This helps you describe the problem in concrete terms - how it shows up in decisions, relationships and routines - and separates problem behaviour from personal identity. Mapping the genealogy of a familiar story is another common move. You and the therapist explore where the story came from - a workplace culture that rewards overwork, family messages about duty, or a training environment that equates worth with endurance.
Gathering unique outcomes is a further practice. These are moments that do not fit the dominant story - times you set a boundary and it held, moments you rested and still cared well, or instances when others stepped in. These exceptions are treated as evidence that alternative stories already exist. Re-authoring conversations collect and enlarge those exceptions so they can form a different, more workable account of your values and commitments. Techniques such as re-membering conversations and outsider witnesses can bring other voices into the narrative - colleagues, mentors or peers who recognise different strengths or perspectives - and help make a preferred story more public and practiced.
What to expect in narrative sessions for compassion fatigue
Pace, questions and progression
If you choose to try narrative work, early conversations typically focus on naming and describing the problem in a way that feels separate from you. A therapist who works with narrative ideas will ask questions that invite you to tell how the issue shows up and to unpack the assumptions that sustain it. You might be asked where the story began, who else has played a part in it, and what the problem has done to your relationships and commitments. The pace is often exploratory and conversational - you are invited to give rich descriptions rather than short diagnostic answers.
Later sessions move toward identifying unique outcomes and re-authoring. This is where the therapist helps you gather instances that contradict the dominant story and explores what those instances indicate about your values, skills and preferred life. Some practitioners use structured conversation maps, while others hold more open, reflective dialogue. It is important to note that most therapists who list narrative therapy among their approaches combine it with other methods. How much of any session is explicitly narrative varies by practitioner. Asking, in an initial call or email, how much of their work is narrative and what a typical session looks like will give you a clearer sense of process and fit.
Is a narrative approach the right fit for compassion fatigue?
Who tends to be drawn to narrative work
People are often drawn to a narrative approach when they do not want to be reduced to a diagnosis or when a story handed down by family, workplace or culture no longer feels true. If you prefer to explore meaning, identity and the social contexts that shape exhaustion, a narrative approach may feel resonant. Narrative work foregrounds your values and how you want to live, which can be especially useful when compassion fatigue is bound up with beliefs about duty and care.
Compared with approaches that focus primarily on thought content - such as cognitive behavioural work - or on skill-building - such as dialectical emphasis on behavioural change - narrative practice places the story about the problem at the centre. It also differs from approaches that emphasise insight into early life events in a psychodynamic frame by prioritising how current stories are formed and can be revised. Many therapists bring together narrative methods and elements from other approaches to address practical needs - for example combining re-authoring conversations with strategies to manage immediate stress. There is no way to tell in advance whether one approach will suit you; most people use an early conversation to find out whether the therapist's style and focus feel helpful.
How to choose a therapist who works narratively with compassion fatigue
Questions to ask and how to evaluate fit
When you look through listings, focus on how therapists describe their focus areas, approaches, credentials, languages and experience. In a first conversation you might ask directly what proportion of their work draws on narrative practice, what a typical session involves, and how they weave narrative ideas together with other methods. It is also reasonable to ask where they hold professional registration or licensure and to verify those credentials with the appropriate local body. Remember that a listing of narrative therapy among approaches is not evidence of a specific qualification in that method - it simply indicates the clinician works with those ideas.
Fit often becomes clear in a short exchange. Notice how the therapist responds to your description of the stories that feel stuck or oppressive, whether they attend to your values, and whether they invite examples of times you acted differently from the dominant story. Narrative work is conversational and relational - it often translates well to video sessions because the focus is on telling, exploring and revising stories together. If you prefer a different pace or more directive skills work, ask how they might integrate other strategies into the work. The goal of the first conversations is practical: to learn how the therapist understands compassion fatigue narratively and whether that understanding resonates with you.
